The Village of Mount Prospect partnered with CHC Wellbeing to create a program that rewards you for your healthy lifestyle choices.
Who is eligible to participate?
- All regular, full-time & part-time eligible employees regardless of enrollment in a Village medical plan,
- Spouses on the Village's medical plan are not eligible to participate in the rewards program; however, they must screen in order to be eligible for the medical discount,
- All conditions of participating in the wellness rewards program need to be met for the rewards program.
The wellness rewards program begins April 27, 2026 and participants can earn points in order to reach reward goals through March 31, 2027. See the chart below for a description of the reward structure.
If a biometric screening is not completed by June 5, 2026, then access to the wellness rewards program will be closed.
View the Rewards Program Guide to see all the activities which could be earning you points towards an additional $50 or $200. (Rewards are paid out after the conclusion of the program. Eligible employees must be actively employed with VOMP at time of payout to receive their reward).
Employees on the Village's medical plan that satisfy the conditions of participating would receive medical insurance premium discount:
- Single coverage receive a 10% discount on medical premiums.
- Single+1 coverage receive a 20 % discount on medical premiums.
- Family coverage receive a 30% discount on medical premiums.
