2026 Wellness Program (Biometric Screening)
WHO CAN SCREEN?
- All regular, full-time & part-time employees (regardless of enrollment in a Village medical plan; excludes interns, volunteers and other seasonal staff) are eligible to participate in our annual biometric screening and rewards program.
- Spouses who are enrolled in the Village's medical plan are also eligible to screen (required for spouses enrolled in the Single +1 or family medical plans in order to receive insurance premium discount).
WHY SCREEN?
- Receive your FREE confidential results (37 panel evaluation with option to purchase additional tests) to alert you of possible health issues and to prevent illness.
- This is your annual opportunity to qualify for the Village's medical insurance premium discount, which could save you up to $172 per year (Single Premium), up to $735 per year (Single + 1 Premium) or up to $1,431 per year (Family Premium) when annualized.

Onsite Screenings
Onsite screenings are a convenient way to get your health journey started. CHC Wellbeing's team of phlebotomists come to a Village location to perform a blood pressure check and blood draw.
-Monday, June 1, 2026 - Public Works, Emergency Operations Center (EOC) - testing will take place from 7:30 a.m. to 12:30 p.m.;
-Wednesday, June 3, 2026 - Village Hall, Farley Community Room - testing will take place from 8:30 a.m. to 1:30 p.m.;
-Friday, June 5, 2026 - Police Headquarters (911 E. Kensington Rd.) - testing will take place from 7:30 a.m. to 12:30 p.m.
Appointments are not needed but recommended. Please go to www.labcorp.com to find a facility nearest to you and to set an appointment. Remember, you have to register with CHC Wellbeing first in order to access the paperwork you need for your LabCorp screening. Visit a LabCorp facility by Friday, June 5, 2026.
Physician Screenings
If you are unable to attend an onsite event or remote screening, you may complete your screening with your physician instead.
Just register for the physician screening option and upload your lab results from your doctor's office to your CHC Portal by logging on to app.chcw.com (click "Complete the Wellness Screening" card on your My Wellbeing Journey page; then click the "Upload Results" button to add your file and submit results) or via fax to CHC Wellbeing's Confidential Fax Number: 847-437-2775 by June 5, 2025. (DO NOT FAX YOUR RESULTS TO HUMAN RESOURCES).
The lab results must included the following five (5) attributes: glucose, triglycerides, total cholesterol, HDL cholesterol and LDL cholesterol. Additional recommended attributes are identified on pages 8 & 9 in the 2026 Screening guide.
All screenings and health risk assessments must be completed by Friday, June 5, 2026 in order to be eligible for the medical insurance discount and the CHC Wellbeing Rewards Program.
WHEN WILL I RECEIVE MY DISCOUNT?
The discounted rate is effective July 1, 2026 and lasts until June 30, 2027 based on the following:
- Employees with single coverage will receive a 10% discount on medical premiums if the employee completes the CHC Wellness screening and health risk questionnaire.
- Employees with single + 1 coverage will receive a 20% discount on medical premiums if the employee and spouse (on the Village's medical plan) complete the CHC Wellness screening and health risk questionnaire.
- Employees with family coverage will receive a 30% discount on medical premiums if the employee and spouse (on the Village's medical plan) must complete the CHC Wellness screening and health risk questionnaire.
***Child dependents do not screen***
