As such, it is the policy of the department to work with those who are seeking a U visa and desire a law enforcement certification as part of the U visa application process. Specifically, victims or their representatives may reach out to the Mount Prospect Police Social Worker to arrange for completion of the Form I-918, Supplement B, U Nonimmigrant Status Certification.
